Special Needs Alert Program (SNAP) Information Packet
The Special Needs Alert Program (SNAP), developed by Delaware's EMSC Program in 2004, is now available in English & Spanish. SNAP is a pre-hospital notification program for children with special health care needs (CSHCN). When a child is registered in the program, first responders are alerted to the child's complete medical history. The prior knowledge of the medical history enables first responders to provide prompt and appropriate emergency care for the child. A home visit by EMS providers may be conducted as a means to introduce families to their local providers and to gather additional information.
